Apicoectomy is a surgical procedure to amputate the tip of the tooth root due to infection.
It is a surgery performed under local anesthesia, in which a window is made in the gum to cut the affected root apex and scrape the surrounding contaminated bone. The root remnant is then sealed with a mineral trioxide aggregate and the tissue (gingiva) is repositioned and sutured closed.
When should an apicoectomy be performed?
When despite a first conventional root canal or endodontic treatment, discomfort persists because it failed to eliminate the infection, then it is necessary to perform an apicoectomy, provided it is the only problem of the tooth; otherwise, if there is horizontal or vertical fracture of the root, carious root remnant and other signs, it is necessary to extract the tooth in a short time because the apical lesion can extend along the bone over the apices of the other roots.
